Clozapine is the most effective antipsychotic in the treatment of refractory schizophrenia. Regular blood monitoring is mandatory during clozapine treatment because of the risk of blood cell side effects such as neutropenia and the more severe agranulocytosis. Unfortunately there is no way to determine if the reaction is mild or severe, and many patients have to discontinue treatment unnecessarily.
