An audit of Dialog / Dialog+ completion within the CMHT.
DIALOG+ is an intervention which involves routine structured conversations between patients and service users and healthcare professionals, such as care co-ordinators. Regular keyworker-patient interactions are an integral aspect of community mental health care. However, until recently, there has been limited research on how to best conduct these meetings in a therapeutically effective manner. This project aims to evaluate how successfully DIALOG+ is being implemented within a single adult community mental health team against the Trust standards and identify any contributing factors that may the impact number of completed DIALOG outcome measures service users receive
