Advance choice documents (ACDs) are statements of preference for mental health care and treatment. ACDs are made when a service user has capacity to complete one together with their treatment mental health professionals and others as desired (e.g. carers or advocates). The project aims to evaluate the implementation of Advance Choice Documents. We will use tables and documents from the electronic health records to evaluate the completion, access, honouring and review of Advance Choice Documents.

The Clinical Record Interactive Search (CRIS) system is a computer system that allows researchers at the NIHR Maudsley Biomedical Research Centre (BRC) to carry out research using information from South London and Maudsley NHS Foundation Trust clinical records.
