Advance Choice Documents Implementation (ACDI): Work package 4: Retrospective study of the contents of Advance Choice Document (ACD) to evaluate key aspects of ACD implementation: Completion, access, honouring, and review.

Advance choice documents (ACDs) are statements of preference for mental health care and treatment. ACDs are made when a service user has capacity to complete one together with their treatment mental health professionals and others as desired (e.g. carers or advocates). The project aims to evaluate the implementation of Advance Choice Documents. We will use tables and documents from the electronic health records to evaluate the completion, access, honouring and review of Advance Choice Documents.
