Physical Health Communiation with GPs (CQUIN - PartB)

  • Applicant: Tom Werner
  • Project ID: 15-086

Cardio-vascular disease is the most important cause of the reduced life expectancy for patients with severe mental illnesses. Recent guidance requests General Practitioner to review the physical health of their patients with psychotic illnesses at least annually. Mental health services can support GPs in this task. Care Programme Approach (CPA) is meant to develop a comprehensive care plan that focuses on both physical and mental well-being. Two-way communication of care plans is an important first step for effective and preventative health planning. This aspect of GP communication is also taken up by our commissioners and included in the second part of the Trust Quality premium (Physical Health CQUIN – Part B GP Communication).
