Full Physical examination withing 24 hours of admission and recorded in patient notes. (100%) Temperature, height, weight, BP and pulse to be recorded on discharge. Must be recorded in notes. (100%) Notification of discharge and report to GP within one week of discharge hwich includes details of physical health. (95%) Recovery model – cases discharged in Q2 – review where the term “patient goals” have been used, tracked.
